29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) Worksite: On or about 8/01/2023, employees were exposed to fall hazards during residential roofing from an approximate height of 21 feet above the lower level, without any means of fall protection.

(a) Worksite: On or about 8/1/2023, employees were accessing a residential roof using a 32 foot Werner extension ladder with its rails set unequally on the downward slope of a pitched roof, causing it to be unstable, thus exposing employees to fall hazards.
