29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On August 1, 2023, on the north side of the construction site, employees engaged in framing activities without fall protection were exposed to unprotected fall hazards exposing employees to a fall hazard of approximately 10 feet to the ground below.

29 CFR 1926.25(a):During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. On August 1, 2023, employees were working at a job site where wooden debris with protruding nails sticking up were strewn all around the residential lot exposing employees to foot injuries and impalement hazards.
