29 CFR 1910.219(f)(3):  Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:   a) On or about May 16, 2023, in the roundtable and miter saw bench area, employees were performing production and cleanup activities near the custom-built trash conveyor with an unguarded chain and sprocket within 7-feet of the floor.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours:  a) On or about October 10, 2023, the employer failed to provide copies of the injury and illness records to an authorized representative.   Barrel 53 Cooperage DBA Midwest Stave Exchange, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.40(a), which was contained in OSHA inspection number 1445288, Citation 2, Item 1 and was affirmed as a final order on 07/10/2020, with respect to a workplace located at 178 Archer Rd. DENNARD, AR 72629.

On or about May 16, 2023, at Barrel 53 Cooperage dba Midwest Stave Exchange, LLC, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2023.   On or about May 16, 2023, an employee suffered an injury that resulted in an amputation resulting in days away from work and medical treatment beyond first aid.
