On or about August 3, 2023, employees were not protected from being struck by forklifts while the forklift operators are pushing up to 4 stacks of pallets in the forward direction around the warehouse at a height that locks the vision of the operator.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or about August 3, 2023, located at the tear down line, employees were not protected from contacting the unused portion and the cutting portion of the blade on three Wood-Mizer PD200 Pallet dismantler band saws while the employees were dismantling wood pallets.  b. On or about August 3, 2023, located at the tear down line, employees who were sorting pallet pieces on the Trim Saw were exposed to an unguarded shear point between the pallet pieces, the conveyor teeth and the jig block mounted on the surface of the table.

29 CFR 1910.213(r)(4): The mention of specific machines in paragraphs (a) thru (q) and this paragraph (r) of this section, inclusive, is not intended to exclude other woodworking machines from the requirement that suitable guards and exhaust hoods be provided to reduce to a minimum the hazard due to the point of operation of such machines.  a. On or about August 3, 2023, located against the eastern wall on the north end of the warehouse, employees were not protected from contacting the blade of the company manufactured chop saw as a result of a four inch square hole in the lower section of the hood guard facing the operator.  b. On or about August 3, 2023, located approximately 15 feet east of the office complex in the warehouse, employees were not protected from contacting the blade of the company manufactured chop saw as a result of a four inch square hole in the lower section of the hood guard facing the operator.

29 CFR 1910.305(g)(1)(iv):Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  29 CFR  1910.305(g)(1)(iv)(A):As a substitute for the fixed wiring of a structure;  a. On or about August 3, 2023, employees were exposed to electrical shock and fire hazards while working at the miter saw cutting station due as a result of an extension cord used to provide 120 VAC power to the station. The temporary wiring was in use as fixed wiring.
