29 CFR 1904.4(a): Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that is work-related, is a new case, and meets one or more of the general recording criteria of 29 CFR 1904.7 or the application to specific cases of 29 CFR 1904.8 through 29 CFR 1904.12. On or about August 4, 2023, at the establishment located at 905 Main Street, Wausaukee, Wisconsin; the employer failed to record a work related illness on the OSHA 300 Log for calendar year 2023 that met recordable criteria. On or about July 9, 2023, while working in the store, an employee sustained a loss of consciousness due to heat illness, and the employer failed to record it on the OSHA 300 log.
