29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width, but the use of a safety monitor alone: a) Job site 3687 California Avenue, Pittsburgh, PA: On or about August 4, 2023, during roofing work on a low sloped roof, the employer did not ensure that employees used appropriate fall protection systems/devices where exposed to fall hazards of up to approximately 25 feet.

29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, were not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) Job site 3687 California Avenue, Pittsburgh, PA: On or about August 4, 2023, during roofing work on a low sloped roof, the employer did not ensure that guardrail systems were of adequate height where fall hazards of up to approximately approximately 25 feet existed in that the parapet walls surrounding the roof were determined to be between 26 and 36 inches tall above the working level.
