29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:   a) Job site, at or near 4019 Cambronne Street, Pittsburgh, PA: On or about June 11, 2024, during roofing activities, the employer did not ensure that employees used appropriate fall protection systems/devices where fall hazards of up to approximately 25 feet existed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its' top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, at or near 4019 Cambronne Street, Pittsburgh, PA: On or about June 11, 2024, during roofing activities, the employer did not ensure that a ladder, used to get up/down to and from the rear porch roof was properly set up in that it only extended up to about 1 foot above the roof line.
