DBA Andersen Windows and Doors located at 1331 South Chillicothe Road Aurora, OH 44202, failed to ensure periodic inspections of the energy control procedures for various machinery were conducted at least annually to ensure that the procedure and the requirement of this standard were being followed. These machineries include but are not limited to the Forklift Battery Charger, Cardboard Baler, Kickoff Booth and Mixers.
