29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: West Warehouse / Trash Compactor Chute Dock: On or about August 10, 2023, employees were exposed to trip / fall-in hazards into a USA Hauling Company #40C-514 trash compactor made up of rotating parts that create nip points and flying debris, for an extended period of time and frequency.

29 CFR1910.110(f)(2)(i): LP gas container(s) in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons: West Warehouse / Shipping & Receiving Dock: On or about August 10, 2023, sufficient protection of liquid petroleum (LP) gas containers had not been provided by employer so as to prevent excessive temperature rise, physical damage, or tampering by unauthorized persons. Multiple PIT (Powered Industrial Truck) LP tanks were left unsecured and unattended in areas involving moderate foot, forklift, and/or commercial vehicle traffic. A) At time of inspection, 2x PIT LP tanks were staged between two loading / unloading dock doors. B) At time of inspection, 1x PIT LP tank was staged and unsecured outside, behind multiple trash containers, located between the LP storage cabinet and a loading / unloading dock. C) At time of inspection, the LP storage cabinet was unsecured with approximately over a dozen PIT LP tanks inside. D) At time of inspection, a standard BBQ grill LP tank was staged unsecured on a storage rack inside the shipping & receiving area with containers of unknown content.
