29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: A) JT Construction on or about August 14, 2023 at the address of 7014 East State Street, Hermitage, PA: Employees were utilizing a fiberglass ladder to access the roof of the structure. The ladder was only two-feet above the surface of the roof. The ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.
