29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  On or about August 15, 2023 and at times prior thereto, in the basement of the facility, large piles of sand and rocks cover the walking working surface and ramp/stairs leading to the basement.  This exposes employees to trip and fall hazards.

On or about August 17, 2023 and at times prior thereto, Caterpillar forklift #3451 was observed in service with a broken front and rear windshield.  This exposed the employees to struck-by hazards.     b.)	On or about August 17, 2023 and at times prior thereto, Caterpillar forklift #3452 was observed in service with a broken rear windshield. On or about August 17, 2023, by the middle transfer table, employees were in use of a large floor fan with openings greater than a half inch on the fan blade cover.  This exposed the employees to a caught-in hazard.         b.)	On or about August 17, 2023, in the basement, employees were in use of a large floor fan with openings greater than a half inch on the fan blade cover.  This exposed the employees to a caught-in hazard.

29 CFR 1910.305(a)(1)(i): Metal raceways, cable trays, cable armor, cable sheath, and other metal enclosures for conductors were not metallically joined together into a continuous electric conductor:    a.)	On or about August 17, 2023, the raceway on the bottom of the middle transfer table was pulled away from its enclosure.  This exposed the employees to an electrical hazard.         b.) 29 CFR 1910.305(g)(1)(iv)(A):  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:                                                                                                                            a.)	On or about August 17, 2023 and at times prior thereto, by the middle transfer table, an extension cord (flexible cord) was being used as a substitute for fixed wiring and was wrapped around other cords and parts of the structure.  This exposed employees to electrical hazards.         b.)	On or about August 17, 2023, in the basement of the facility, an extension cord (flexible cord) was secured to parts of the structure. This exposed employees to electrical hazards.    c.)	On or about August 17, 2023,  in the Pug Mill, an extension cord (flexible cord) was secured to parts of the structure.  This exposed employees to electrical hazards.

On or about August 17, 2023, a metal cased heat lamp was missing the ground prong on the lamp cord. This exposed the employees to an electrical hazard.
