29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) On or about August 22, 2023, located at N94W17937 Appleton Avenue, Menomonee Falls, WI 53051,  the employer did not ensure the extension ladders that employees were using to access the roof on site was extended 3 feet above the eave of the roof, exposing employees to a fall hazard of approximately 13 feet.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about August 22, 2023, located at N94W17937 Appleton Avenue, Menomonee Falls, WI 53051, on a 6:12 pitch roof employees were stacking shingles on the upper ridge without the use of fall protection, exposing the employees to a fall hazard of approximately 30 feet to the ground below.

(a) On or about August 22, 2023,  located at N94W17937 Appleton Avenue, Menomonee Falls, WI 53051, employees were stacking shingles on the lower landing without the use of fall protection, exposing the employees to a fall hazard of approximately 13 feet to the ground below.
