29 CFR 1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. a) On or about 08/23/2023, throughout the warehouse, employees were exposed to struck-by/crushed-by hazards when the employer did not ensure a Powered Industrial Vehicle (PIV) Training Program, consisting of practical training and evaluations, was in place. Employees operate PIVs such as, but not limited to: Toyota Electric Forklift Model 8FBCU25 Serial #61589. Powered Industrial Vehicles are used to move materials throughout the warehouse. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: a) On or about 08/23/2023, in the warehouse, the electrical panels on the south side of the building were blocked by forklift charging stations, exposing employees to contusions if there was an emergency that required shut off at the electrical panel. NO ABATEMENT CERTIFICATION REQUIRED
