29 CFR 1910.23(d)(12)(ii): The employer did not ensure that the step-across distance from the centerline of the rungs or steps for side-step ladders, is not less than 15 inches (38 cm) and not more than 20 inches (51 cm) to the access points of the platform edge. a) Arc Melting Caster Area - Employees used a side-step ladder to access the top of the tundish and the step across distance measured approximately 26.5 inches wide, on or about August 23, 2023. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: a) Arc Melting Caster Area - Employees were permitted to walk and work on top of a tundish at heights of approximately 58.5 inches high up to 79 inches high while performing inspection duties without being protected from falling, on or about August 23, 2023. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
