29 CFR 1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a. On or about August 25th, 2023, in the US Housing and Urban Development Office, Computer Room, two fire extinguishers, Halotron, model 11SA Halotron, were on the floor, and not properly identified. b. On or about August 25th, 2023, in the US Housing and Urban Development Office, in the Reception Area, one fire extinguisher, dry chemical, model 10HISA80ABC was obstructed by a desk and without a sign to identify them. These situations exposed employees to fire hazards. NOTE: Certification and documentation of abatement must be submitted for all instances of this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a.) On or about August 25th, 2023, in the US Housing and Urban Development Office, inside the Administrative Office's Interview Room, there was a relocatable power tap connected to another forming a daisy-chain. b.) On or about August 25th, 2023, in the US Housing and Urban Development Office, between the cubicles of two employees (Damian E. Ongay and Jennifer Rodriguez), an extension cord connected to a relocatable power tap. There was also another relocatable power tap connected to that extension cord as noted in instance (a). above st mentioned before. These conditions can exposed employees to electrical and fire hazards. NOTE: Certification and documentation of abatement must be submitted for all instances of this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a. On or about August 25th, 2023, at the US Housing and Urban Development Office, Vacant Administration Cubicle, there was an extension cord connected to a shredder. Part of the cover of the extension cord protecting the internal cables was damaged. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
