29 CFR  1926.1404(d)(1)(i): 29 CFR 1926.1404(d)(1)(i): Before commencing assembly/disassembly operations, the assembly/disassembly (A/D) director did not ensure that the crew members understood all of their tasks:  On or about 08/26/2024, at the west side of HCA Mercy Hospital's entrance, employees not involved in the lift were exposed to crush-by hazards due to the crew members not understanding their tasks, which led to only one of two pins being deployed or extended through the bore holes of the jib.

29 CFR  1926.1412(c)(1): Upon completion of assembly, the equipment was not inspected by a qualified person to assure that it is configured in accordance with manufacturer equipment criteria:  On or about 08/26/2024, at the west side of HCA Mercy Hospital's entrance, employees not involved in the lift were exposed to crush-by hazards due to the qualified person not conducting an inspection of the pins to ensure they were deployed through the bore holes on the jib.

29 CFR 1926.1425(a): Where available, hoisting routes that minimize the exposure of employees to hoisted loads were not used, to the extent consistent with public safety:  On or about 08/26/2024, at the west side of HCA Mercy Hospital's entrance, employees not involved in the lift were exposed to crush-by hazards due to lack of hoisting routes while working and walking in and around the fall zone when hoisting operations were in progress.
