29 CFR 1910.23(b)(8): Ladders are used only for the purposes for which they were designed. On or about August 24, 2023, with regard to an establishment located at 2216 Calumet Drive, New Holstein, WI 53061, employees were exposed to fall hazards while they were allowed to use an extension ladder to access a fixed ladder on the Fly Ash Silo when the manufacturer of the ladder prohibits going from one ladder to another.

29 CFR 1910.28(b)(3)(iv):(iv) Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. On or about August 7, 2023, with regard to an establishment located at 2216 Calumet Drive, New Holstein, WI 53061, employees were exposed to fall hazards when fixed ladders, such as the ladder leading to the water boiler, were not equipped with a self-closing gate or offset to prevent falls through the opening of the guardrail system.
