29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a. On or about August 31, 2023, employees were exposed to stuck-by hazards, when required to operate Hyster forklift No. 9, Model S155FT with Serial No. D024V01744E. The power industrial vehicle oil leak and was not removed from service.  b. On or about August 31, 2023, employees were exposed to stuck-by hazards, when required to operate Hyster forklift No. 10, Model S155FT with Serial No. F024V01690K. The power industrial vehicle oil leak and was not removed from service.  c. On or about August 31, 2023, the employees were exposed to struck by hazards, when required to operate Hyster forklift No. 8 Model: S70FT with Serial No. H187V15284U. The power industrial vehicle did not have a functioning horn and was not removed from service.  d. On or about August 31, 2023, the employees were exposed to struck by hazards, when required to operate Hyster forklift No. 4, Model S80FTBCS with Serial No. G004V03431F. The power industrial vehicle has its mast lowering on its own and was not removed from service.  e.  On or about August 31, 2023, the employees were exposed to struck by hazards, when required to operate Hyster forklift No. 3, Model S80FTBCS with Serial No. G004V03430F. The power industrial vehicle has cracks on the clamp attachment and was not removed from service.   Roosevelt Paper Company was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.178(q)(1), which was contained in OSHA inspection number 1635839, citation number 1, item number 2 and was affirmed as a final order on March 30, 2023, with respect to a workplace located at 5100 West 123rd Street, ALSIP, IL 60803.
