29 CFR  1926.503(a)(2)(ii):The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used.  (a) On or about Sept 6, 2023, at the jobsite located at N49W6327 Western Road, Cedarburg, WI 53012, the employer did not ensure that the correct procedures for erecting fall protection were followed with anchors that were not rated (rated up to 12:12 when used over the peak) for a 14:12 pitch roof.  In addition to using the anchors against manufacturers specifications the anchors themselves only had four to five nails on each side of the anchor (required six per side when using over the peak). This exposed employees to a fall hazard of approximately 21 feet.
