29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a) On or about November 16, 2023, at the jobsite located at N49W6337 Western Road, Cedarburg WI, employees were performing roofing work with pneumatic nail guns without the use of eye protection.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) On or about November 16, 2023, at the jobsite located at N49W6337 Western Road, Cedarburg WI, employees were performing roofing work on a three story apartment building with a pitch of 12:12 and a fall height of approximately 30 feet. Employees connected lanyard and rope grab to the D ring attachment point located on the side of the full body harness and not in the center of the wearer's back near shoulder level.
