29 CFR 1910.119(d)(3)(i)(H): Process safety information pertaining to the equipment in the process did not include the safety systems (e.g., interlocks, detection or suppression systems): The employer failed to maintain accurate process safety information for the safety systems and interlocks on the raw flake conveyance systems to the Extractor. The process safety information documented a safety interlock for an air actuated slide gate at the discharge of the Extractor Plug Screw when conveying goes down although this piece of equipment was not interlocked to close when conveying was stopped.

29 CFR 1910.119(e)(3)(iii): The Process Hazard Analysis did not address engineering and administrative controls applicable to the hazards and their interrelationships such as appropriate application of detection methodologies to provide early warning of releases. (Acceptable detection methods might include process monitoring and control instrumentation with alarms, and detection hardware such as hydrocarbon sensors.) On or about September 10, 2023, and at times prior, the PHA did not address and implement effective engineering and administrative controls to prevent Hexane from entering into the Raw Flake L-Path that was interconnected into the Prep Building.

Addressing instances where exterior doors were propped open which would prevent the ventilation system from capturing fugitive Hexane from leaving the process into unprotected areas outside of the process; b. Addressing instances where standard operating procedures were not being followed in sequential order to prevent Hexane from entering upstream connected equipment in the Prep Building.
