29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a worksite located at 8101 SE Holly Drive, Blue Springs, MO, an employee was exposed to fall hazards while using a portable extension ladder. The employer exposed the employee to these fall hazards by allowing them to use a portable extension ladder that was not positioned to allow the side rails to extend more than 3 feet above the upper landing surface being accessed.
