29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log within seven (7) calendar days of receiving information that a recordable injury or illness occurred: a) On or about September 13, 2023, and at times prior, the employer did not correctly document the following workplace injuries on the OSHA Form 300 or its equivalent for the calendar year 2023 within seven (7) calendar days of receiving information of a recordable injury or illness occurrence. Violative instances included, but were not limited to, Employee 2 (case #23_200954) sustained a loss of consciousness on July 21, 2023. The worker was not listed on the original OSHA 300 Log provided September 13, 2023.
