On or about September 27, 2023, in the Deacro area of the warehouse, the employer did not remove and/or replace defective rigging equipment being used by employees rigging paper rolls. As a result, employees were exposed to a struck-by crushed-by hazard.

29 CFR  1910.184(c)(14):Employers must not use slings without affixed and legible identification markings.  a.)On or about September 27, 2023, the employer failed to ensure that slings used by employees in the Deacro Are of the warehouse had the required affixed and legible identification marking. As a result employees were exposed to struck-by and crushed-by hazards from an inability to verify suitability and safety information of the slings being used for rigging paper rolls.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a.)On or about September 7, 2023, the employer did not ensure that the Winder 15 (Jenner Jahn) conveyor system, located in the warehouse, was adequately guarded in that pinch points and caught between hazards were present along the conveyor frame. As a result, an employee was caught and injured.

29 CFR  1910.219(f)(3):Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a.) On or about September 7, 2023, the employer did not ensure adequate guarding of the chain and sprocket system on the Winder 15 (Jenner Jahn) conveyor system in the warehouse. As a result employees were exposed to caught in and pinch point hazards from unguarded chain and sprocket mechanism that moved pallets along the line.

As a result, employees were exposed to a caught-by and entanglement hazard.     b.) On or about September 7, 2023, the employer did not ensure horizontal rotating shafts, on laminator Line 2 in the warehouse, which were less than seven feet from the floor were adequately guarded. As a result, employees were exposed to a caught-by and entanglement hazard.
