29 CFR 1910.184(c)(9):Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s): On or about August 17, 2023, at 1049 Griggs Road, Jefferson, OH 44047, the employer failed to ensure employees lifting die sets in the die department were protected from crushing injuries in that employees manipulate die sets with their hands and/or feet while suspended. An employee had an approximate 350 pound die fall on their foot while attempting to flip the die block.

29 CFR 1910.184(e)(3)(i):A thorough periodic inspection of alloy steel chain slings was not made on a regular basis and/or were done at intervals greater than once every 12 months: On or about August 17, 2023, at 1049 Griggs Road, Jefferson, OH 44047 the employer failed to ensure employees were protected from crushing hazards by ensuring thorough inspections were performed on the chain slings in the die department in that the lifting pins were wore down and did not fit in the dies properly which contributed to a die falling on an employees foot.

29 CFR 1910.184(e)(1):Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: On or about August 17, 2023, at 1049 Griggs Road, Jefferson, OH 44047, the employer failed to ensure the chain sling located in the die room utilized to lift dies had a tag attached. Employees utilize the chain sling to lift 350 pound dies up to move around the work area.
