29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about April 9, 2025, the employer did not ensure the development, documentation, and utilization of proper energy control procedures to manage potentially hazardous energy during maintenance activities on the MC3 forging machine. Specifically, while employees were engaged in replacing the hollow mill heads located at the rear of the machine, there were no documented procedures in place to control hazardous energy sources. This oversight exposed employees to significant risks of amputation hazards due to the potential for unexpected activation of the machine or the release of stored energy.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 09, 2025, the employer did not ensure adequate machine guarding for the MC3 forging machine, exposing employees to serious hazards such as crush injuries and entanglement with moving parts, including pallet clamps and rotating pallets. The existing light curtains designed to secure the loading zone are inadequate, extending only 8 inches above the two-hand swiping device. This design flaw permits operators to reach into the danger area while the machine is in operation, significantly increasing the risk of workplace injuries.
