29 CFR 1910.95(c)(1): The employer did not administer a continuing and effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) Leefson Tool & Die Company located at 850 Henderson Boulevard, Folcroft, PA 19032 - Press Shop Area - The employer did not administer a continuing effective hearing conservation program for employees performing metal stamping activities and exposed to continuous noise levels equivalent to 83.4%, 70.73%, and 75.56% of the allowable 50% dose for an 8 hour time-weighted average (TWA). These exposures were equivalent to an 8 hour TWA of 88.6 dBA, 87.5 dBA, and 87.9 dBA respectively, exceeding the OSHA Action Level of 85.0 dBA for an 8 hour TWA. Sampling times were 467, 461, and 459 minutes, respectively. Zero exposures were assumed for all unsampled time periods. Violation noted on October 30, 2023.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: a) Leefson Tool & Die Company located at 850 Henderson Boulevard, Folcroft, PA 19032 - On or about September 18, 2023, a written Hazard Communication Program was not provided where employees are exposed to hazardous chemicals while operating stamping and CNC machines, and when chemicals are applied to metal products before and after they are run through machines. Employees are exposed to hazardous chemicals including but not limited to the following: -Hydrotreated heavy paraffinic distillate and sulfurized fatty esters, as contained in Stamp Lube #155-C. -Aliphatic hydrocarbons, as contained in Vanishing Oil #101. -Distillates, hydrotreated heavy paraffinic and residual oils, solvent-dewaxed as contained in Zurnpreem 95A. -etc.
