29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Employees performing clearing obstruction operations were exposed to amputation and electrical shock hazards in that the Dryer 4 Baghouse Airlock was not locked out prior to clearing obstructions. The employer had not ensured that the airlock had been locked out.
