29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Jobsite located at 3612 Haverford Ave., Philadelphia, PA.: An employee was in a trench digging out the concrete footers. The employer did not ensure employees working in the class b soil trench which measured approximately 80 inches in depth, 64 inches in width, and 64 inches in length were protected from cave-ins by an adequate protective system. Condition noted on or about September 22, 2023.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. a) Jobsite located at 3612 Haverford Ave., Philadelphia, PA.: An employee was working in a trench to dig out concrete footer. The employee did not have adequate means of egress in that a ladder was not readily available in the trench and was only placed in the trench when the employee was ready to exit. Condition noted on or about September 22, 2023.
