Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. a) On or about September 22, 2023, the employer did not conduct frequent and regular inspections of the job site. As a result, employees working in the interstitial space above the factory floor were exposed to a fall hazard of 20 feet in that light fixture holes were not covered.

29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a) On or about September 22, 2023, the employer failed to ensure that all light fixture holes in the interstitial space above the factory floor were covered for employees working in the area installing power drops to the equipment below. As a result, an employee was hospitalized after they fell 20 feet through a light fixture hole.

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) On or about September 22, 2023, the employer did not ensure that each employee, exposed to a fall hazard, was trained on the hazards of falling and the procedures to minimized the hazards in the work area. AS a result, an employee was hospitalized after they fell approximately 20 feet through a light fixture hole.
