29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: A) In the quality assurance cage area, the employer used flexible extension cords to power office workstations that were set up inside the cages. The flexible cords ran through holes in the chain links and were run around sharp metal corners where damage could occur over time. Some flexible cords showed signs of damage, wear, and tear. Damaged flexible cords create fire and shock hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: A) In the quality assurance cage area, the employer provided power for workstations with flexible extension cords that were plugged into wall outlets. The cords were not temporary wiring. Use of flexible cords as permanent wiring creates a fire hazard.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment were not installed and used in accordance with any instructions included in the listing or labeling. A) In the quality assurance cage area, a power strip was plugged into another power strip (daisy chained) in order to power equipment and appliances in chain link metal cages used as offices by the employer. Use of connected multi-plug power strips creates a fire hazard.
