On or about September 28, 2023, at the address of 4080 Bennett St, Toledo OH 43612: The employer did not ensure an employee operating a forklift was adequately trained in the operation of power industrial vehicles. An evaluation of competency for the employee operating the forklift was not conducted and documented by the employer. Due to this condition, the operating employee and others working around the forklift were exposed to struck-by hazards.
