29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) On or about October 5th, 2023, employees at 568 High Rd., Glen Ellyn, IL 60137, were exposed to danger of head injury from impact and falling objects while working underneath a shingle hoist.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about October 5th, 2023, employees at 568 High Rd., Glen Ellyn, IL 60137, were exposed to unprotected fall hazards in excess of 10 feet during residential roofing activities.  Lagunes Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA Inspection number 1447351, Citation 1, Item number 1 and was affirmed as a final order on September 14th, 2020, with respect to a workplace located at 724 N West Street, Wheaton, IL 60187

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:    On or about October 5th, 2023 employees at 568 High Rd., Glen Ellyn, IL 60137 were exposed to unprotected fall hazards in excess of 10 feet during residential roofing activities. The employer did not ensure they were adequately trained to recognize and minimize the hazards associated with falls.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).  Lagunes Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA Inspection number 1447351, Citation 1, Item number 2 and was affirmed as a final order on September 14th, 2020, with respect to a workplace located at 724 N West Street, Wheaton, IL 60187

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 28th, 2023, employees at 568 High Rd., Glen Ellyn, IL 60137 were exposed to an approximate 10 foot fall while using a portable ladder that did not extend three feet past the landing surface.  Lagunes Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA Inspection number 1447351, Citation 1, Item number 3 and was affirmed as a final order on September 14th, 2020, with respect to a workplace located at 724 N West Street, Wheaton, IL 60187
