29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a. On or about May 23, 2025, at the above addressed jobsite, employees working on the ground, under a worker performing residential roofing operations, were not wearing a hard hat to protect against falling materials and debris. Lagunes Construction Corp. was previously cited for a violation of this occupational safety and health standard 1926.100(a), which was contained in OSHA inspection number 1701468, citation number 1, item number 1 and was affirmed as a final order on December 20, 2023, with respect to a workplace located at 568 High Rd., Glen Ellyn, IL 60137.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. On or about May 23, 2025, the employer failed to certify that employees had been trained to recognize and minimize fall hazards. Employees were exposed to fall hazards of seventeen (17) feet eight (8) inches, above the ground, while performing roofing activities on a residential structure without properly using fall protection.
