29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a. South Pack Room, #2 Tank and #13 Holding Tank - On or about October 2, 2023, the employer failed to ensure an energy control procedure was developed and documented for the control of hazardous energy when an employee was engaged in a service and/or maintenance task that included clearing a transfer line through the use of air pressure to shoot a "PIG bullet" through the line. As such, the transfer line was not depressurized prior to an employee disconnecting the line from the #13 Holding Tank, exposing the employee to hazards associated with the rapid depressurization of the line when disconnected.
