On or about October 4 and preceding in the 185 Kitchen at Colusa Casino Resort, employees in the dishwashing area were not provided mats where wet work was being performed. Employees were exposed to slip and fall hazards.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling. #1: In the maintenance shop, on top of the black cabinets, there were 3 sets of relocatable power taps daisy-chained together. Employees were exposed to electrical shock and fire hazards. #2: In the maintenance shop, 2 relocatable power taps were secured to building fixtures and used as a substitute for permanent wiring to energize radio chargers. Employees were exposed to electrical shock hazards. #3: In, the laundry room, a microwave was plugged into a relocatable power tap.
