29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas. On or about October 5, 2023, an employee of a subcontractor under the control of the General Contractor performed masonry activities around an area where scrap boards with nails protruding were left on the ground in areas where employees were working. This exposed the employees to stepping on the boards and creating puncture and laceration hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffolds, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. On or about October 5, 2023, On or about October 10, 2023, an employee of a subcontractor under the control of the General Contractor performed masonry activities and was not provided a safe means of access and egress to the upper levels of a scaffold and was exposed to fall hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. On or about October 5, 2023, a subcontractor's employees, under the control of the General Contractor, were engaged in masonry activities working from a scaffold. They were not provided adequate fall protection while performing these tasks from a scaffold more than ten feet above the ground. As a result, the employees were exposed to fall hazards greater than ten feet.
