29 CFR  1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there was an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees.  On or about October 10, 2023, PRECISION ROOFING & REMODELING LLC did not require employees to wear foot protection such as puncture-resistant boots when exposed to puncture and laceration hazards while roofing and clearing debris. Nails were embedded in shingles on the roof and debris on the ground in a worksite at 1334 Heritage Parkway in Wentzville, Missouri. Employees were allowed to wear tennis shoes that did not protect their feet from puncture and laceration hazards.

On or about October 10, 2023, PRECISION ROOFING & REMODELING LLC did not ensure that each affected employee who was exposed to eye hazards from flying particles while operating a pneumatic nail gun used appropriate eye protection at a worksite at 1334 Heritage Parkway in Wentzville, Missouri.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about October 10, 2023, PRECISION ROOFING & REMODELING LLC was repairing the roof of a residential home located at 1334 Heritage Parkway in Wentzville, Missouri.  The employees were exposed to fall hazards.  They were not protected by guardrail systems, safety net system, or personal fall arrest system while working more than 12 feet above the ground.

The program had not enabled each employee to recognize the hazards of falling and had not trained each employee in the procedures to be followed in order to minimize these hazards.  On or about October 10, 2023, PRECISION ROOFING & REMODELING LLC did not provide training to each employee who was exposed to fall hazards. The employees were roofing a residential house located at 1334 Heritage Parkway in Wentzville, Missouri. Employees were not able to recognize the hazards of falling and were not trained in the procedures to be followed to minimize fall hazards.

29 CFR 1926.59:  The employer had not developed a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) as incorporated by reference:  29 CFR 1910.1200(e)(1): Employers had not developed, implemented, and maintained at each workplace, a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets (SDSs), and employee information and training would be met.  On or about October 10, 2023, PRECISION ROOFING & REMODELING LLC had not developed, implemented, and maintained at each workplace a written hazard communication program that at least described how the criteria specified in paragraphs (f) for labeling, (g) for maintaining SDSs, and (h) for employee information and training would be met. Gas was utilized in a worksite at 1334 Heritage Parkway in Wentzville, Missouri, to power air compressors for the use of the pneumatic nail guns.
