29 CFR  1910.212(a)(3)(iii):Special handtools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone:  (a) At a job site located at 24110 S Peculiar DR, Peculiar, MO; employees were exposed to caught-in-between hazards while operating CF-506 Carson end former.  Employees operating this machine use a special hand tool for placing products into the machine but the tool did not prevent the operator from placing their hand/fingers in the danger zone of the clamping action of the machine.  This resulted in an amputation on or around October 4, 2023.
