29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  At 4209 Tower Ln, Crowley, Texas: On or about October 11, 2023 and at times prior thereto, employees engaged in framing activities without fall protection were exposed to unprotected fall hazards of approximately 11 feet to the ground below.   The Jose Angel Vazquez Rivera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1653972, citation number 1, item number 1 and was affirmed as a final order on  03-31-2023, with respect to a workplace located at 4227 Tobin Dr. Crowley, TX 76036.

29 CFR  1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.   On or about October 11, 2023 and at times prior thereto, the employer did not provide an extension ladder where employees could access and egress the residential building upper levels, exposing them to potential fall hazards.  The Jose Angel Vazquez Rivera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1051(a), which was contained in OSHA inspection number 1653972, citation number 1, item number 2 and was affirmed as a final order on  03-31-2023, with respect to a workplace located at 4227 Tobin Dr. Crowley, TX 76036.

29 CFR  1926.403(b)(1):Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees.  On the north side of the construction site, employees installing framing material used a ROLAIR air compressor and the electrical plate was missing the cover plate exposing the internal wiring were exposed to electrical hazards.
