29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a)B&B Contracting, Inc. at Hospital Drive, Webster, SD 57274: On or about and at times prior to October 11, 2023, the controlling employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the jobsite, materials, and equipment; conducted by a competent person. Subcontracted employees were exposed to egress and cave in hazards as a result of conducting commercial trenching activities. This condition exposed subcontracted employees to egress and cave-in hazards.
