29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  140 Everett St., Easthampton, MA: On or about October 12, 2023, an employee was observed using a ladder that did not extend 3 feet past the upper landing surface of the roof they were accessing.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  140 Everett St., Easthampton, MA: On or about October 12, 2023, employees were observed working on a residential structure engaged in roofing activities with unprotected sides and edges greater than 9 feet above the ground below and were not protected by either guard rails, safety net systems, or personal fall arrest systems, exposing them to fall hazards.  HDZ Construction Inc, was previously cited for a violation of an equivalent Occupational Safety and Health Standard, 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1523157, citation number 1, item number 1 and was affirmed as a final order on September 20, 2021, with respect to a workplace located at 619 Sumner Ave., Springfield, MA.
