29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On and/or before October 11, 2023, employees conducted multiple tasks such as sweeping, brass deformation, and removal of animal feces at a regular basis. The employer did not enforce the use of safety glasses during the operations. Employees were potentially exposed to biological and other solid projectile objects which can cause severe eye irritation, corneal abrasions, punctured retina, or partial/permanent blindness.  Notice 1, Item 11, 29 CFR 1910.133(a)(1) as required by 29 CFR 1960.8(b): The employer failed to enforce the use of safety glasses to protect employees from eye hazards working in immediate distance of flying particles. The employer was cited for a violation of the same standard on June 16, 2022, Inspection number 1585683, July 14, 2022, of final order date and means of abatement.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.  Brass Deformation Area: On and/or before October 11, 2023, employee(s) conducted brass deformation operations that contained spent/deformed shell casings with multiple metal compositions and were required to wear a 3M N95 particulate respirator. The employer did not provide a medical assessment to determine the employees' ability to wear the designated respirator for assigned tasks. Employee(s) were exposed to hazards that could exacerbate underlying medical condition.   Notice 1, Item 6-a, 29 CFR 1910.134(e)(1) as required by 29 CFR 1960.8(b): The employer failed to provide a medical assessment to determine the employee's ability to use a 3M N95 respirator, before the employee was fit tested or required to use the respirator in the workplace. The employer was cited for a violation of the same standard on June 6, 2022, Inspection number 1585683, July 14, 2022, of final settlement and means of abatement.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator.   Brass deformation area: On and/or before October 11, 2023, employees conducted brass deformation operations that contained spent/deformed shell casings with multiple metal compositions and were required to wear a 3M N95 particulate respirator. The employer did not provide documentation showing initial fit tests were conducted for respirators used. The employees were exposed to respiratory irritations, and lastly, potential damage to the nervous systems from mixed metal dusts.  Notice 1, Item 7, 29 CFR 1910.134(f)(2) as required by 29 CFR 1960.8(b): The employer failed to ensure an annual fit test had taken place for the employee(s) utilizing 3M N95 particulate respirator at this workplace. The employer was cited for a violation of the same standard on June 16, 2022, Inspection number 1585683, July 14, 2022, of final settlement and means of abatement.

The employer did not collect and remove wastes regularly to maintain a sanitary workplace. Employees were exposed to rodent borne diseases such as Hantavirus Pulmonary Syndrome.
