29 CFR  1910.303(b)(5):Circuit impedance and other characteristics. The overcurrent protective devices, the total impedance, the component short-circuit current ratings, and other characteristics of the circuit to be protected shall be selected and coordinated to permit the circuit protective devices used to clear a fault to do so without the occurrence of extensive damage to the electrical components of the circuit. This fault shall be assumed to be either between two or more of the circuit conductors, or between any circuit conductor and the grounding conductor or enclosing metal raceway.  (a) at 26568 Redwood Highway, Cave Junction, Oregon , Research Building, on or about October 12, 2023 and at times prior thereto: Employees were exposed to electric shock and burns hazard from an faulty GFCI electrical outlet , resulting in serious electrocution, burn injuries and disabilities.

29 CFR  1910.157(e)(2):Portable fire extinguishers were not visually inspected at least monthly:  (a) at 26568 Redwood Highway, Cave Junction, Oregon , Research Building, on or about October 12, 2023 and at times prior thereto: employees were exposed to potential fire hazard which could result in serious injuries or illness due to all portable fire extinguishers in the recreation bay area did not have  monthly maintenance inspections performed. Note:  Applies to any extinguishers provided in the workplace.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.157(e)(2), which was contained in OSHA inspection number 1594183, notice number 1, item number 1a and was affirmed as a final order on8/18/2022, with respect to a workplace located at Forest Service, Parkdale Work Center.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.157(e)(2), which was contained in OSHA inspection number 1261318, notice number 2, item number 6 and was affirmed as a final order on 3/20/2018, with respect to a workplace located at Mt. Hood National Forest.

(a)  at 26568 Redwood Highway, Cave Junction, Oregon , Research Building, on or about October 12, 2023 and at times prior thereto: Employees were exposed to serious eye injuries from unserviceable eyewash  located in Kitchen sink area, for First-Aid use in the event of a splash and emergency exposure to an irritant chemical to the eyes, which could be aggravate the employee's condition, the eye wash was disassembled and could not be a source of a potential eye flush resulting in serious eye infection or irritation.
