29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system. At the jobsite, employees removing old shingles and installing new underlayment to the roof of the house, were not wearing fall protection, exposing the employees to an unprotected fall hazard at a height of over 6 feet to the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. At the jobsite, where employees performing roofing activities were exposed to fall hazards up to approximately 28 feet and did not demonstrate hazard recognition and avoidance to fall hazards at heights over 6 feet to the ground below.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. At the residential construction site, the employer did not ensure employees installing roofing material were trained to recognize and avoid slip and fall hazards related to ladders.
