29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) Industrial Sales and Manufacturing, on or about 10/12/23 and times prior thereto: Employees did not lockout prior to servicing operations on equipment including but not limited to, saws, grinders, and presses. Employees are periodically required to conduct servicing operations including but not limited to, blade changes, abrasive wheel replacement, and/or tool changes. Employees were not applying lockout devices and/or maintaining exclusive control. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(7):The employer shall provide training to ensure that the purpose and functions of the energy control program are understood by employees and that the knowledge and skills required for the safe application, useage, and removal of the energy controls are acquired by employees. A.) Industrial Sales and Manufacturing, on or about 10/12/23 and times prior thereto: Employees designated as affected are performing authorized employee tasks and have not received authorized training. Tasks include but are not limited to changing dies, blades, and grinding wheels B.) Industrial Sales and Manufacturing, on or about 10/12/23 and times prior thereto: employees who perform service/maintenance tasks on equipment have not received training on the concept of exclusive control. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Grinding equipment, including, but not limited to Gallmeyer & Livingston Surface Grinder and Dayton 8" Bench Grinder.
