29 CFR  1910.151(b): Medical and First Aid. Medical services and first aid. In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which is used for the treatment of all injured employees, a person or persons shall be adequately trained to render first aid. Adequate first aid supplies shall be readily available.  a) On or about September 27, 2023, medical service or first aid was not provided when an employee was injured while working on Gloucester 64" Dual Spindle Winder - Portable Unit when the safety knife being utilize got stuck and entangled within the film wrap injuring employee's arm within the roll. The employer sent the injured employee to the break room instead of calling 911; injured employee was in the break room for approximately twenty-five (25) minutes before a co-worker volunteer to take injured employee to a near-by hospital for treatment.

29 CFR  1910.212(a)(1): General requirements for all machines. Machine guarding. Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips, and sparks. Examples of guarding methods are barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about September 27, 2023, the employer did not ensure that machine guarding was provided on the Gloucester 64" Dual Spindle Winder - Portable Unit machine exposing employees to a caught in hazard created with the constant rotation of the plastic film being rolled onto spindles.  The Crayex Corporation, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1467647, citation number 1, item number 3 and was affirmed as a final order on 05/27/2020, with respect to a workplace located at 1747 Commerce Drive, Piqua, OH 45356.  The Crayex Corporation, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1532775, citation number 1, item number 1 and was affirmed as a final order on 09/22/2021, with respect to a workplace located at 1747 Commerce Drive, Piqua, OH 45356.

29 CFR 1904.39(a)(2): Reporting fatalities, hospitalizations, amputations, and losses of an eye as a result of work-related incidents to OSHA. Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a) On or about September 27, 2023, the employer failed to report to OSHA an accident within twenty-four hours that resulted in a hospitalization of an employee. OSHA-Cincinnati Area Office received a phone call from injured employee to let us know about the hospitalization.
