29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. a) Package Press: On or about October 10, 2023, and times prior, employees stood on a platform to use the bundle bander while operating the package press. Employees were exposed to falling or trip hazards due to the platform not being secured to prevent tip over within inches of a machine with unguarded moving parts.

Employees were not adequately trained on the use of the equipment and hazards of the area.
