29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth: Retaining wall 2, where connection to 30 inch water pipe began: An employee assigned to work in the excavation with a face of 6 feet, helping an excavator operator fill in soil and assigned to protect a communication conduit running through the excavation, did not have safe access, such as a ladder, acceptable sloped earth, ramp, or other safe means of egress. Employees were exposed to fall hazards.

29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation. Retaining wall 2, where connection to 30-inch water pipe began: An employee was protecting communication cable conduit in an excavation measuring 6 feet deep in which there was accumulated water. An employee was exposed to crushed-by and struck-by hazards.
