29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Building B; Packout Department: On or about October 4, 2023, and at times prior, the employer exposed employees to amputation hazards, in that machine guarding for the PL5 Cutter, DM&E Model 60 Serial No. 250 was not provided to employees while they crilled/rethreaded tuff strands between the press wheel and cutter wheel with the cutter door open.
